HOURS OF REGISTRATION Monday- Friday 9:00am to 12:00pm

NEW REGISTRATIONS TO MDCPS MUST PROVIDE THE FOLLOWING:

AGE & LEGAL VERIFICATION

Must provide one of the following:

Original Birth Certificate or
Passport/Certificate of Arrival in the U.S. showing age (only if born in a country other than U.S.A.)

VERIFICATION OF ADDRESS

Must provide two of the following:

Electric bill, showing name and address of parent
Broker's or attorney's statement of purchase of residence
Properly executed lease

HEALTH REQUIREMENTS

Must provide both of the following:

Student Health Examination - DH3040 physical examination must be performed less than a year old
Florida Certificate of Immunization - DH680 from a private doctor or local heath provider

SCHOOL RECORDS

Must Provide the following:

Verification of credits earned for grade placement; and interpretation of foreign records at no cost available from the Federal & State Compliance Office

TRANSFERS FROM ANOTHER MIAMI-DADE COUNTY PUBLIC SCHOOL

Parent/legal guardian must bring a withdrawal slip from sending school
Proof of address with name of parent/guardian. (Electric bill)
